At IWK Health Mental Health & Addictions, we use Measurement-Based Care to better understand how patients are doing throughout their care journey.
Measurement-Based Care involves tools that help patients share how they're feeling, what's improving, and where they may need additional support. We use a secure digital platform called Greenspace Health to collect this information and help care teams review results over time. The feedback we receive helps guide conversations between patients and care providers and supports decisions about care.
Measurement-Based Care is one of a number of tools and approaches that our care teams use to better-understand each client's experiences, strengths, needs, and goals.
How it works
At different points during care, patients may be invited to complete short questions designed to be quick and easy to complete. The questions may ask about:
- Daily functioning and well-being
- Progress toward personal goals
- Mood and emotions
Patients and their care providers also have flexibility to use measurement-based care to track other items that they feel may be relevant to the patient's care.
Responses are reviewed by the patient's care team and discussed with clients to help inform ongoing care.
How it helps
Measurement-Based Care helps us:
- Better understand patient needs and experiences
- Track progress over time
- Identify concerns sooner
- Support meaningful conversations between clients, families, and care providers
- Make informed decisions together about care
Improving Care, Together
Measurement-Based Care helps ensure that patient and family experiences remain at the centre of care. The information shared by patients helps us understand what is working and identify opportunities to better support their well-being and care experience.
In addition to supporting individual care, Measurement-Based Care helps us better understand how services are performing across the Mental Health & Addictions program. By looking at trends and outcomes over time, we can support quality improvement, service planning, evaluation, and research - helping us continuously strengthen services and better meet the needs of children, youth, and families.
