Are you between the ages of 13 and 15 and have arthritis, chronic pain, a previous cancer diagnosis, diabetes, cystic fibrosis, or any ongoing health issues?
Will you need to transfer to the adult healthcare system? Did you know that was a thing?
Eventually, most youth with special health care needs treated in a pediatric setting will need to transfer to adult healthcare for ongoing treatment. The education on transitioning should start early.
The You’re in Charge workshop was designed for youth with a chronic health condition, aged 13 to 15 and their parents or caregivers to build self-management skills and confidence. Through these workshops, youth will learn the skills to independently manage their chronic health conditions and navigate the health care system. The program provides information on the transition process and how to prepare. Youth will also learn from peers who have gone through the transition process and now receive care in the adult system.
Parents, this is for you too! Our program is family-focused. We require at least one parent/caregiver to attend with their youth to help build independence, skills, health history and learn about the transition process. Parents and caregivers will have the support of their peers on what has worked for them and share about helping their teens take on more responsibility for their healthcare.
Workshops Offered
This two-day workshop is offered for free through Zoom for Healthcare. The first day of the workshop will teach about self-management and transition. On the second day, youth and parents will break into groups. Through activities, discussions, resources and tools, we will explore managing medications, health history, and a current summary of health. We will talk about goal setting, and everyone will create an action plan with their goals.
