Community Health Teams
Community Health Teams (CHTs) are a service of IWK Health and Nova Scotia Health. We offer free one-on-one navigation services and free group wellness programming. Our team of healthcare professionals help people across the lifespan to improve their health and wellness. This flexible service does not require a referral and you can attend in person or join virtually. Interpretation services are available.

Group Wellness Programs
We provide health information, resources, and strategies to make health behaviour changes. Topic areas include Parenting, Youth Wellness, Healthy Eating, Mental Wellness, Physical Activity, and Reducing Your Health Risks. Understanding and getting connected to the right support can be complicated. IWK Mental Health & Wellness Coordinators offer children, youth, and families personalized support through our Wellness Navigation service. Our team can help you connect you to the right resources and overcome health barriers.
- Are you a parent with concerns about your child's mood or behavior?
- Are you a youth looking for support in connecting to recreation and physical activity programming?
- Would your family benefit from support with housing, medication coverage, or food security?
- Are you having difficulty navigating the health system or finding community resources for your child?
If you or your family want help, you can meet with us by phone, Microsoft Teams, or in person. This service is available to those living in the greater Halifax area with a valid Nova Scotia health card. Call 902-460-4560 and select your area to make an appointment.
