Within the Children’s Health Program, the Discharge Planning office works in conjunction with pediatric nursing units and outpatient clinics to provide families with a resource for their many needs while at home caring for their child.
How we help
The Discharge Planning office provides guidance and access to equipment and supplies ensuring families have the tools they require to be at home. This includes:
- Enteral (tube) Feeding
- Ventilator
- Tracheostomy
- Home Total Parenteral Nutrition (TPN)
- Bleeding Disorders
- Continuing Care
- Oxygen Saturation Monitoring by Pulse Oximeter
Our office can also help you with any of the following tasks:
- Support with Continuing Care Services
- Researching product availability
- Making arrangements to order products
- help in securing any possible funding
