We are working to make the move from the IWK to adult health care better. We call this ‘transition to adult care’. It includes helping patients and families prepare for adulthood, coordinating a transfer of care so everyone knows what is happening and making sure people stay connected to adult care.
The IWK has a Transition Framework to help us create a more structured process to support transition. We are using this framework to help us as part of a research study. Over the coming months, we want to see if using this framework helps to improve the transition experience for youth in three different IWK clinics.
We have a survey to help us measure how this work is helping people as they get ready for adult care.
Who can do the survey?
- IWK patients (14 years old and over) diagnosed with Sickle Cell Anemia, Thalassemia, Inflammatory Bowel Disease, Diabetes or a Congenital Heart Condition
- Caregivers/parents of IWK patients (14 years old and over) diagnosed with one of these conditions
What is the survey about?
- The survey asks questions about managing your health care (or your child’s health care). We want to compare responses at the beginning and the end of this study.
How long will it take to do the survey?
- It will take approximately 15 minutes to complete the study
As a thank you, once you have completed the survey, you can be entered into a draw for one of thirty gift cards (valued at $25).
