If you have a chronic condition and you’ve transferred to Adult Care, you have knowledge and experience that could really help other families more easily make the transition to adult care.
If you’re interested in sharing your experiences, are comfortable speaking and leading activities in a small group, enjoy working with youth, and can use virtual programs like Zoom, then the “You’re in Charge” program could use your help!
How it works
You’re in Charge is being offered through Zoom for Healthcare. The workshops will run as a two-part series. Sessions will run based on participant registration and needs.
