If you have a youth with a chronic condition who has transferred to Adult Care, you have knowledge and experience that could really help other families more easily make the transition to adult care.

If you have a chronic condition and you’ve transferred to Adult Care, you have knowledge and experience that could really help other families more easily make the transition to adult care.

If you’re interested in sharing your experiences, are comfortable speaking and leading activities in a small group, enjoy working with youth, and can use virtual programs like Zoom, then the “You’re in Charge” program could use your help!

We need patients and families like you to help us understand what your needs and priorities are when it comes to care and service, and to give us your perspectives in what we do well, and what we could be doing better. We are looking for Partners from all backgrounds, age groups, and experiences. Parents, guardians, current patients, former patients, siblings, grandparents, partners, sons, daughters, other support persons. All your voices are important and needed.

We are so thankful for the incredible generosity of our community and the many baby hats, finger puppets, comfort dolls & quilts we’ve received! Your support truly makes a difference to our patients and families.

At this time we will be pausing the acceptance of in-kind donations for the next year. This will help us manage our current inventory more effectively and ensure that every item we receive can be put to its best use.
